Case Management workers offer short-term, goal-oriented services to improve mental health, quality of life and opportunity to work towards recovery * includes life skills training, community support and advocacy for people who have a serious mental illness

- coordination of care and services
- group programs for skills development and social and recreational activities
- health education about mental disorders and use of medications
- relapse prevention planning and strategies
- referral and advocacy services to other community agencies or partners
