Program is focused on helping patients continue their recovery safely and confidently at home * take a whole-person approach, supporting not just medical needs but overall well-being and independence * goal is to make the transition home smooth, coordinated, and focused on helping patients regain strength in the environment where they feel most comfortable

Assists to continue your recovery at home instead of staying in the hospital longer than necessary. Once medically ready to leave, the team arranges the supports that an individual needs so they can focus on getting stronger in their own space.

A Southlake@home Coordinator stays in touch with individual and their care team, making sure services are in place and ther transition home feels smooth. Depending on the needs, an individual may receive rehabilitation (physiotherapy or occupational therapy), nursing care, or personal support at home. The program centres around rehab and recovery—building confidence, mobility, and independence day by day. Southlake@home provides support for up to 16 weeks, and when ready and help connect the individual to ongoing community services, if needed.
