Admissibilité / population desservie
People 12 years and older * admitted patient or recently discharged from London Health Sciences Centre * willing to be screened and assessed * deemed medically stable * experiencing concerns related to substance use and/or withdrawal, gambling or gaming and would benefit from support to meet immediate needs upon discharge * must be facing barriers to connecting with addiction resources and supports on their own

Program facilitates discharge process from St Thomas Elgin General Hospital * ensures patients have linkages to appropriate addiction and mental health services and supports upon discharge * patient works with Addiction Transitional Case Manager to create and implement plan for care
Forensic Transitional Case Management - Facilitates and supports the transition of inpatients of forensic units through creating and supporting community placements * assessment, treatment and rehabilitation of people with mental illness who have been in significant contact with the law * support with transition to the community, counselling, follow-up, crisis intervention, and linking to outpatient community mental health agencies * works in collaboration with the Forensic Outreach Team of Southwest Centre for Forensic Mental Health Care
