Description des services:

Works in partnership with primary health care providers to offer services for and raise awareness of Hepatitis C virus (HCV) risks, care and treatment

Provides a treatment team made up of HCV Treating Physicians, a Treatment Nurse and a Case Coordinator

Services and support include:

Primarily community based client care
Raising awareness of testing options and access to HCV testing
Access to care and treatment
Access to mental health and addiction counselling
Education and awareness of risk factors and prevention
