Description des services:
Telehomecare program links patients from home to registered nurses at Brampton Civic. Using telephone and internet communication, as well as vital signs monitoring devices, nurses monitor and support people to manage conditions such as heart failure or chronic obstructive pulmonary disease and chronic heart failure
