Self-referral or referral by Physician or Nurse Practitioner required

Individuals 15 years of age and older and a resident of NW Ontario
Have a Neurological diagnosis (stroke, acquired brain injury, MS, ALS, Parkinson's Disease, or spinal cord injury
Require professional intervention
Actively participate in therapy for a minimum of one hour per visit
Be independent with toileting or have caregiver assistance

A goal-oriented outpatient neurology rehabilitation service for clients with neurological conditions

Helps clients maximize independence and their functional potential
Assists clients to independently access community resources and maintain their ability to live at home
Provides the option of having individual or group sessions
Offers a team of healthcare providers, including: Occupational Therapist, Physiotherapist, Psychological Associate, Rehabilitation Assistant, Social Worker, Speech-Language Pathologist, and Therapeutic Recreation
