Through a partnership with the Central West Self-Management Program (CWSMP), Osler provides support for people with chronic health conditions, including diabetes * workshops are group-based and non-clinical
Peer-led group sessions include chronic disease self-management, diabetes self-management and chronic pain self-management. Each six-week session runs for two and a half hours, once a week

At this time, sessions are offered online only through Zoom to residents of the Central West region (Brampton, Etobicoke, Dufferin-County, Caledon, Malton, Woodbridge and Rexdale) * Online programs can be brought to community groups and organization

- setting personal goals
- creating realistic action plans
- identifying and solving common problems
- managing pain and fatigue
- developing healthy eating and physical activity plans
- managing depression, stress and difficult emotions
- managing medications
- working with health care providers
