Admissibilité / population desservie
Adults 18 years of age and older who are:
- living with a serious mental illness
- able to live with others in shared spaces which may include a shared bedroom
- willing to participate in program activities
- willing to work with a Community Homes for Opportunity worker
- willing to complete annual income verification paperwork
Frais
Rent geared-to-income (rent includes meals) * annual income verification paperwork required * source of income (employment or social services support such as Ontario Works or ODSP) required or eligible for rent support

Description des services:
Voluntary, person-centered supportive housing program for people with serious mental illness * assists people by providing appropriate housing and support services to achieve and maintain stability in a home that is safe and affordable * program takes a recovery approach and provides access to support services both in the Community Homes for Opportunity (CHO) home and in the community
Program offers:
Homes that are now part of the CHO program were once a part of the Homes for Special Care (HSC) program
Program offers:
- housing, meals, and 24-hour supportive care services
- works to improve and or stabilize physical and mental health, foster independence and promote integration into their community
- one-on-one and group support in home and in the community
- support in learning/enhancing basic life skills, activities of daily living and interpersonal skills
- support with medication and symptom management
- support to identify and work towards recovery goals
- referrals to other community resources
