Qui sommes-nous ?
To enhance patient self-management and provider health literacy, partnering with primary care as well as community based health and social service providers, we offer evidence-based trainings and programs for community members and health care providers. Annually, we serve 1200 people and this work takes place in partnership with 37 organizations across the city.
