Provides off-site supervision and support to member communities in their implementation of the Community Wellness Worker (CWW) program

Program delivered at the community level consists of the following services:

Provides assistance for individuals to identify areas of concern and to develop action plans to address concerns
Liaises and networks with related health and social agencies and provides referrals
Provides crisis intervention education and workshops
Offers illness prevention and wellness planning, information and education
Provides community outreach education to promote cultural awareness
