Outpatient services for seniors
Nurse-Led Outreach Team (NLOT) - Nurse practitioners at Osler partner with long-term care homes in the community to provide timely care to seniors and avoid unnecessary transfers to hospital
Geriatric Outreach Team (GOT) - Multidisciplinary team (serving patients living in Ontario Health atHome, Central West area) that specializes in caring for seniors living in the community * in-home non-urgent geriatric assessments for frail seniors who are unable to attend clinic visits * assessments focus on functional, psychosocial, cognitive and physical health status
