  • Physician referral required for clients admitted from acute care and/or the community

  • If an individual doesn't have a family physician, referrals can be made by a walk-in clinic doctor, community health clinic, the local hospital, or Ontario Health atHome - North West Area

Admissibilité / population desservie

Seniors 65 years of age and older who are experiencing complex health problems, including

  • Multiple admissions to the hospital or multiple visits to the emergency department

  • Cognitive changes

  • Changes in health status or functional decline


Note: Individuals must be medically stable to participate in the program and have the cognitive capacity as well as potential stamina and endurance to participate in daily therapies

Description des services:

  • Provides a low to medium intensity rehabilitation program to help older adults improve their abilities to live in their own homes with or without support
  • Focuses on the clients medical, functional and cognitive needs in order to optimize their well-being
  • Refers individuals to additional supports, such as a Psychologist, Psychiatrist, Registered Dietitian, Speech Language Pathologist, or Spiritual Care Associate if needed
  • Works with Ontario Health atHome - North West Area to facilitate discharge back to the community with formal support services
