Works with individuals experiencing symptoms of mental illness who require support to live and work in the community * case managers provide on-going support as needed in order to stabilize, achieve goals, and improve quality of life * Case Management services are tailored to the individual, based on their identified goals for support * participation in the Case Management Program is a prerequisite to obtain CMHA Huron Perth supportive housing

Services offered include, but are not limited to:

• goal setting and support with treatment planning
• advocacy
• education and resources about living with mental illness
• referral and connection to various community services
• support engaging in activities of daily living while managing mental wellness
• connection to in house addiction services
