Eligibility / Target Population

Older adults 65 years and older, with long-term or late-onset mental illness, or severe behavioural disturbances
Regional Psychogeriatric Program: Older adults with behavioural or mental health issues in the ten counties of Southwestern Ontario and rural Middlesex County, including Sarnia/Lambton County

Description of Services

Specialized psychiatric care and support for older adults who suffer from late onset mental illnesses or severe behavioural responses * program focuses on assessment, treatment, rehabilitation, prevention, family and community support and education *

Geriatric Day Program
- Offers various professional programs and services including cognitive neuropsychiatry, mood disorders and psychotic disorders clinics, as well as limited therapeutic programs and therapy for outpatients

Discharge Liaison Team
- Facilitates the transition of current inpatients or registered outpatients undergoing transition from hospital to the community

Regional Psychogeriatric Program
- Provides clinical assessments in the patient’s home, community clinic or long-term care facility, for older adults with behavioural or mental health issues
