Provides education and services for the prevention, management and treatment of diabetes

Provides case management and screening services
Offers basic foot care services
Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
Advises on developing and implementing an approach to care and management of diabetes
Assists individuals in finding appropriate services
Provides referrals to other health care professionals and the Northern Diabetes Health Network
Coordinates education and awareness activities for Health Centre staff such as healthy breakfasts, group walking events and health-related videos
