Individuals with Alzheimer's disease or other dementias
Family, friends, and other partners caring for a person with dementia
Individuals concerned about memory loss or other cognitive changes

Description of Services

Links patient and family members through early referrals to community services for non-medical management from the point of diagnosis, throughout the continuum of the disease

Connects individuals to dementia education training sessions

Provides active follow-up and monitoring of patients and their family caregivers to assist in reducing the number of patient crises

Works to reduce caregiver burnout and premature long term care placement

First Link referral partners include:

Community health centres
Geriatric specialists
Health Service providers
Nurse practitioners
Pharmacies
Primary care practitioners
