Description of Services

Links patient and family members through early referrals to community services for non-medical management from the point of diagnosis, throughout the continuum of the disease

Connects individuals to dementia education training sessions

Provides active follow-up and monitoring of patients and their family caregivers to assist in reducing the number of patient crises

Works to reduce caregiver burnout and premature long term care placement
