Eligibility / Target Population
Be a Canadian citizen or landed immigrant at least 18 years of age and have an acquired brain injury, including traumatic brain injury, infection, anoxia, aneurysm, tumor and stroke * must have valid Ontario Health Insurance Plan (OHIP) card * must have free will commitment to rehabilitation and/or participation in Dale Brain Injury Services programs * serious medical issues must be stabilized
Fees
Set fee for individuals with third party funder * free to those who do not have third party funding * occasional costs for specific activities

Description of Services
This program delivers
one-to-one transitional support, rehabilitation and service coordination
to individuals in their home or community * services are flexible and
focus on skills training, improving quality of life and making
connections with other service providers and community resources * more
intensive services are available to support transitions from hospital to
home or long term care * services are available across the South West
and are generally offered from six to 12 months
