Eligibility / Target Population

Be a Canadian citizen or landed immigrant at least 18 years of age and have an acquired brain injury, including traumatic brain injury, infection, anoxia, aneurysm, tumor and stroke * must have valid Ontario Health Insurance Plan (OHIP) card * must have free will commitment to rehabilitation and/or participation in Dale Brain Injury Services programs * serious medical issues must be stabilized

Fees

Varies per program * services are covered by Government funding for most clients * cost of rent, food personal items not covered for those living in a residential program

Description of Services

Provides client-centered services to people affected by an acquired brain injury (ABI) and stroke * services are accessible, evidence-based and responsive to the changing needs of clients, community, and partners to support individuals to maximize their strengths and live healthy, meaningful lives

Focus of rehabilitation is to develop and improve functional skills to increase independence and improve quality of life reflecting best practices * all services are client-centered, strengths-based, least-intrusive, evaluated, and provided in a respectful, ethical manner within a wellness, interpersonal skills, and meaningful activity (WIM) framework and are goal-driven

Intensive Community Transitional Services - Short term, more intensive services delivered to people who are transitioning from hospital to home or long-term care, and/or who require intensive services in order to increase their ability to live as independently as possible in their home environment * services are offered for 6-12 months and are available in London and surrounding areas. This program includes caregiver services.
 
Residential Transitional Services - Short-term community-based assessment and transitional rehabilitation in a structured 24/7 residential environment that includes assessment, skills training, capacity building with the client, family and their support system, and a seamless transition from hospital to home or long-term care * services are offered for 6-12 months and are available in London only
 
Short Term Case Management - Services are designed to quickly respond to individuals requiring immediate supports to prevent or resolve a crisis situation * services available across the South West Region and can be accessed for 6-8 weeks
 
Respite Services - Services are provided either in the home or in DBIS’ residential setting in the London area for individuals who require short-term respite to provide their care partner some time away from their caregiving duties; for clients who are in crisis or for those who live alone and require support while recovering from an illness or medical procedure and whose needs can be met in the program

Counselling - Supports are provided to individuals and/or their caregivers with a focus on understanding acquired brain injury and development of coping strategies * services are provided via face to face sessions, video conference, or teleconference

Caregiver Support Services – Services include residential and in-person respite options, counselling, caregiver support groups, virtual coffee chats, information sessions and a caregiver facebook group * services assist caregivers to take a break, learn self-care tools, get connected to community resources, receive education, support and training to assist in learning about acquired brain injury and how best to interact with and support their loved one and to have an opportunity to connect with other caregivers to share experiences, learn from and support one another * services are available across the South West Region with the exception of residential respite, which is available in the London area only

  • Assisted Living Services Providing 24/7 residential services in a structured, safe environment within the London community * services are flexible and individualized, with a focus on increasing and maintaining skills and enhancing quality of life * service length ranges from short-term to lifelong

  • Community Transitional Services This program delivers one-to-one transitional support, rehabilitation and service coordination to individuals in their home or community * services are flexible and focus on skills training, improving quality of life and making connections with other service providers and community resources * more intensive services are available to support transitions from hospital to home or long term care * services are available across the South West and are generally offered from six to 12 months

  • Consultation and Training Provides general education to care partners and service providers about Acquired Brain Injury (including stroke) in addition to offering support and training to assist individuals and teams in supporting and interacting more effectively with clients who experience behavioral, social and cognitive challenges after acquired brain injury * services are available across the South West Region.

  • Group Services Available across the South West Region, Group Services are flexible and provided in a group setting focusing on individual and group goal achievement, increased independence and quality of life * groups provided offer social, recreational, wellness, skill building, exercise and therapeutic activities * services are available on a short-term, transitional or lifelong basis
