Eligibility / Target Population

Adults 18 years and older with an acquired brain injury (exceptions can be made on a case-by-case basis for people 16-18 years) * must be medically stable * must have potential to improve through rehabilitation process * must be committed to participating and have identified rehabilitation goals that can be met within 12 months * must have tolerance for support sessions lasting at least 60 minutes * must have family/caregiver commitment to participate where applicable

Fees

Free unless third-party insurance involved * occasional costs for specific activities

One-on-one transitional support, rehabilitation, and service coordination for adults with an acquired brain injury * flexible services focused on skills training and improving quality of life * referrals to other service providers and community resources * services delivered in client's home or community * services offered for up to 12 months
