Description of Services

Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible

  • Adult Day Program Adult day program * social, recreational, and therapeutic activities for older adults and adults with disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
  •  
  • Alzheimer and Dementia Services Offers one-on-one support and education to people with Alzheimer’s disease and related dementia and their caregivers * available at the VON office during in-home visits, as well as through regular telephone contact
  •  
  • Blood Pressure Clinic Trained volunteers measure older adult's blood pressure and provide educational materials and information on how to be more heart-healthy at designated areas throughout the county
  •  
  • Caregiver Support One-on-one support and monthly caregiver support groups for caregivers of people with Alzheimer's disease and related dementia
  •  
  • Caregiver Training, Information and Education Peer support groups, education, and training for family members, service providers, and caregivers of people with Alzheimer's disease or a related dementia * caregiver resource library available

  • Congregate Dining Group meals and social activities for older adults and adults with disabilities

  • Cool-Aid Clients receive assistance in completing medical information in a provided template, so it is accessible for emergency personnel in the event of an emergency

  • Home Help Assistance with daily chores, such as laundry, light housekeeping, errands, shopping, meal planning and meal preparation * Home Help personnel visit weekly, bi-weekly, or monthly

  • Hospice and Supportive Care Companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-threatening illness at any stage of illness from diagnosis on * bereavement support for anyone who has experienced the death of someone close

  • Hospice Volunteer Visiting Trained volunteers provide one-to-one support, companionship, and respite for caregivers, and emotional support to people and their family/caregivers dealing with a life-altering chronic or terminal illness

  • Meals on Wheels Volunteers deliver hot or frozen meals to clients to help meet their nutritional requirements * hot meals delivered at lunch * supper available in some locations

  • SMART Exercise Program (Seniors Maintaining Active Roles Together) Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls

  • Telephone Reassurance and Security Checks Regular visits and phone calls to check on the health and safety of older adults and adults with a disability people who live alone * screened volunteers listen, talk, and help isolated people stay in contact with the outside world

  • Transportation Services Non-emergency transportation to local medical and non-medical appointments and regular activities such as shopping, errands, and social outings * volunteer drivers * wheelchair accessible and ambulatory transportation is available in Strathroy-Caradoc, Thames Centre, Aylmer, East Elgin, St Thomas and Central Elgin

  • Volunteer Transportation Non-emergency transportation to local medical and non-medical appointments and regular activities such as shopping, errands, and social outings available throughout Middlesex County for older adults aged 55 years and older or adults living with a disability

  • Volunteer Visiting Regular volunteer visits to older adults aged 55 and older and people with disabilities * volunteer visitors are matched one to one with clients to provide companionship and support and may take place in-home or outside the home
