Description of Services
12-week, specialized in-home support program designed for isolated older adults living with dementia and currently on a waitlist for Adult Day Programs (ADS) in Halton
Program includes:
4 hours per week of personalized in-home care led by trained dementia care professionals, providing enrichment and activity for the client, and respite for the caregiver
customized care plans tailored to each person’s interests and abilities, co-developed with the family
dementia-friendly activity kits for use during and beyond the visits
support and coaching for family caregivers to build confidence and resilience
dementia-friendly social events
