Description of Services

Supports for older adults, and adults with physical disabilities and mental health concerns * helps people remain independent in their own homes for as long as possible
* provides care and support to people who are unable to care for themselves: dementia care, home care, social support, fall prevention, hospice, end of life care and bereavement support * programs to support both the physical and mental wellbeing of people by reducing social isolation and providing care: adult day programs, support groups, nursing care, and friendly visiting programs

  • Community Support Services Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible

  • Patty's Place - Dementia Care Centre Supports for people living with dementia: adult day programs, overnight respite, caregiver support and education
  •  
  • Technology Accessibility Program The program helps seniors to stay connected with friends and family by using technology * participants will receive individual training in their own home, once a week for three months * training is provided by volunteers * tablets are available on loan and access to the internet can be provided, if required
