Description of Services

Medical health centre that provides multidisciplinary care in collaboration with primary care providers in Windsor-Essex for patients with chronic conditions, mild to moderate mental health conditions, and addictions through team-based allied health care

As a program of the Windsor Family Health Team, works towards achieving health and wellness goals * inclusive of Francophone, Indigenous, LGBTQ+, and the multicultural communities of Windsor-Essex * employees receive ongoing training in equitable and inclusive, culturally sensitive, and safe care

We provide all patients 16 and up the opportunity to receive the best care possible to produce optimal outcomes. The team will use their expertise and work with you to develop a plan of care. Together with The Windsor Family Health Team , the team is committed to providing an inclusive and welcoming space to receive care, void of stigma, racism and oppression.
