Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurses, dietitian, and social worker
- mental health and anxiety programs * one-on-one counselling and therapy
- arthritis rehabilitation and education program * occupational therapist coaching and therapy * in-home or in-office * Arthritis Society of Canada sponsored program (open to everyone)
- asthma and chronic obstructive pulmonary disease (COPD) programs * education and self-management
- nutrition and lifestyle programs
- diabetes management programs
- smoking cessation program
- LGBTQ+ culturally sensitive service delivery
- a-fib (atrial fibrillation) program

The Windsor Family Health team offers high quality primary health care for residents in Windsor and the surrounding area. An inter professional group of health care practitioners, including family physicians, provide health care in an integrated, respectful and holistic environment to help keep patients healthy before illness occurs. The focus of the health team is on disease management and prevention, rehabilitation, palliative care and health promotion.
