Referrals must be made by physicians or nurse practitioners. Please fax the completed Centre for Mental Health referral form. Eligibility criteria: Patients whose reason for referral is related to mood disorders (anxiety, depression, bipolar disorder), OCD, PTSD or psychotic disorders (psychosis, schizophrenia, schizoaffective disorder). Additionally, patients with suspected personality disorders seek diagnostic recommendations and potential psychotherapy if appropriate. Patients must reside in catchment area: Parkside Drive to the west, Yonge Street to the east, Lake Ontario to the south and Eglinton Avenue to the north.
