assistance with age-related cognitive, mental health and addiction crises
information and referral to local services and supports
information and referral for community support services such as adult day programs for frail seniors or those with dementias, case management, congregate dining, foot care, home help, hospice care, meals on wheels, medical escort, nursing, respite care for caregiver relief, security checks, shopping assistance, transportation
intake for COSS (Crisis Outreach for Service)
intake point for Behaviour Support Outreach Teams
