Provides community based rehabilitation where staff meet with individuals in their home or community to promote independence in various areas of day to day functioning such as physical, cognitive, behavioral, social, emotional and activities of daily living * services are also provided in a group setting offering various support, educational and leisure opportunities to survivors of a brain injury
