Description of Services
Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses and other health professionals
Focuses on preventing illness and promoting health, and looks at the best way to:
- wound care
- nutrition
- diabetes
- pharmacy
- respiratory program
- occupational therapy
- chiropody services
- cognitive behavioural therapy
- smoking cessation
- women's health
- pap smears for rostered clients
- HPV testing (not paid by OHIP)
Focuses on preventing illness and promoting health, and looks at the best way to:
- manage chronic illnesses such diabetes, asthma and heart disease
- provide better access to mental health services
