Referrals to Cardiac Wellness Program must be made by a physician
Eligibility / Target Population
Participants must be 18 years of age or older and have suffered one of the following events in the past year: heart attack, by-pass surgery, cardiac transplant, angioplasty, stable CAD/angina, valve replacement, congestive heart failure, cardiomyopathy

Services are provided without direct costs upon presentation of a valid OHIP card

Medically managed cardiac rehabilitation program that assists individuals following a cardiac event * program responds to the growing concern of heart disease in the community by providing medical, educational, nutritional, psychosocial and exercise support for people with cardiac related concerns
