Description of Services

Medical care team focused on family and community health * provides acute and chronic disease management, health promotion and disease prevention * team includes family doctors, nurse practitioner, registered nurse, registered dietitian, social worker, occupational therapist, addictions/mental health counselor, pharmacist, and psychologist

Health Promotion and Disease Prevention Programs - Groups sessions, including prenatal classes, open to registered patients and all community members
