Description of Services
Supports patients who need additional behavioural and caregiver support to return home safely. Along with rehab and short-term care, the program includes behavioural strategies and caregiver coaching to help manage challenges at home. The goal is a smooth, well-coordinated transition that promotes safety, confidence, and recovery in the comfort of home.
Designed for patients who are medically ready for discharge home from hospital, but require additional behavioural and caregiver support beyond standard home care.
The program provides behavioural assessment and strategies through LOFT, including coaching for family/caregivers and support managing responsive behaviours in the home. Alongside this, patients continue to receive the same services offered through Southlake@home, such as rehabilitation (physiotherapy and occupational therapy), nursing, and personal support based on assessed needs.
CHATS adds caregiver counselling, community navigation, and wheelchair-accessible transportation for appointments or medication pickups. A Coordinator works with the patient and family to organize services and ensure a smooth transition home.
The program provides up to 16 weeks of focused care, with support to connect patients to ongoing community services if needed.
