Application
Referrals to the program are from the municipality's By-Name List or by Health Sciences North directly. Call or email for information.
To be added to the By-Name list, individuals must be unhoused for a period of at least two weeks and complete an assessment with one of the following agencies: City of Greater Sudbury Social Services (ask for client navigators), Réseau Access Network, the Homelessness Network (community outreach workers), Centre de Santé Communautaire du Grand Sudbury, N’Swakamok Native Friendship Centre, Elizabeth Fry Society, John Howard Society, or the Canadian Mental Health Association.
A City Client Navigator can be seen in person at:
Homelessness division,
199 Larch Street, 9th floor
Monday to Friday 1 pm-4 pm.

Description of Services
Lotus Program is a transitional housing program. Healthcare services are provided for psychosocial rehabilitation for the skills necessary for independent living in permanent housing.
Target clients are individuals who are currently homeless, and motivated to participate in psychosocial rehabilitation with the goal of safe and permanent housing. The following supports are provided housing, primary care from on-call physicians, on-site psychiatry clinics and daily support from nursing, social work and addictions workers.
Transitional housing is based on voluntary participation by individuals.
Upon successful completion of the program, the client will be successful in securing housing and have mastered many of the skills needed to sustain independent living. The client will continue to be connected with the program for follow-up support as needed.
