Description of Services
Release planning case management to non-violent adults with a serious mental illness or serving sentences in the Stratford Jail, Elgin Middlesex Detention Centre, St Thomas Forensic Unit and Central North Correctional Centre
- works with incarcerated individuals and their support person(s) to formulate a plan intended to increase the success of community reintegration
- connection to resources and psychiatric services
- urgent consultation to families, individuals, service providers, particularly police officers, when dealing with an individual who has, or maybe about to commit an offense
