Patients who have been admitted to St Catharine's Site, Niagara Falls Site or Welland Site with Congestive Heart Failure (CHF) or Chronic Obstructive Pulmonary Disorder (COPD) receive supports to assist in the transition from hospital to home with a 60-day pathway of home support
