Description of Services
Education and exercises for people who are recovering from medical treatment related to heart disease * 16 week program
Three options: group program (in person), education only (one to one, in -person or virtual), or home-based (virtual/phone with unsupervised exercise)
Available for all patients of all Cottage Country Family Health Team locations, in Bracebridge, Gravenhurst, Bala, Port Carling, Severn Bridge
