Provides varying levels of support for people who may experiencing homeless, are precariously housed or living in social/supported housing or congregate care settings. Eligibility criteria: - Individuals who are 16 and older living in York Region - Individuals who have a mental health concern, with our without substance use issues, or a developmental disability - Clients may be homeless, precariously housed, or living in social/supported housing or congregate care settings
