Description of Services
The Clinic supports seniors at high risk of injury due to a fall. Assessments include interviews, questionnaires, standardized fall risk and frailty testing, physical examinations and cognitive screening, when appropriate.
This clinic is appropriate for people aged 65 years or older who
Patients are assessed and receive a coordinated care plan from a team of experts
This clinic is appropriate for people aged 65 years or older who
- have a history of falls
- have been at risk of falls in the past year; or
- show clinical signs of frailty (e.g. slower gait speed, weight loss, fatigue, weakness, sedentary lifestyle with or without cognitive impairment, etc.
Patients are assessed and receive a coordinated care plan from a team of experts
