Description of Services

Inter-professional care team of primary care physicians and allied health professionals working together to improve the health and wellness of the South West Mississauga and East Mississauga communities

Programs and services:
  • Access Primary Care Mississauga - for people that do not have family doctor or nurse practitioner, program connects to health care
  • Ontario Structured Psychotherapy (OSP)
  • Primary care nursing services (provide care that promotes health, manages chronic illness and prevents disease)
  • Primary care pharmacist services (run a polypharmacy clinic for all patients with polypharmacy needs (5 or more medications))
  • Health education and promotion (offers information sessions in a group setting covering various topics related to chronic disease prevention and general well-being * sessions are offered virtually * call 905-362-3508 to save a spot or register online)
  • Nutrition counselling (offer individual and group education and counselling for a variety of health conditions and nutritional needs including diabetes, heart health, plant-based diets, obesity, gastrointestinal disorders and eating disorders)
  • Mental health and social service supports (provide access to mental health and addictions support to promote optimal mental health, strengthen management of illness and optimize prevention)
  • Cancer screening clinic (offers cancer screening services to anyone in Ontario who does not have a family doctor or nurse practitioner)
  • Memory clinic (help patients with memory difficulties, dementia, and Alzheimer’s care)
  • Chiropody clinic (provides patient education and individualized foot care services)
  • Physiotherapy clinic (provides assessment, education and treatment of acute or chronic conditions to patients who are otherwise unable to access physiotherapy through OHIP, benefit or private pay community based services)
  • Community vaccination clinics (seasonal flu shots, COVID-19 vaccines)
  • Chronic disease management (team-based approach in managing diabetes, heart disease, high blood pressure, COPD, etc.)
  • Smoking Cessation Program (help adults quit smoking or reduce nicotine use)
  • Social service supports (navigational support to community resources and assistance programs to improve quality of life)
