Description of Services
Community-based health care in which paramedics provide alternative to family doctor, walk-in clinic, or emergency department * provides preventative, at-home health care visits * focuses on reducing avoidable use of emergency and ambulance services * helps older adults and vulnerable, at-risk people live safely in their homes * services include:
- health and medical risk assessments
- chronic disease management
- medication management
- care coordination with primary care providers and other agencies
- referral to community programs and resources
- remote patient monitoring
- point of care testing (blood draws/tests, urinalysis testing, INR tests, and A1C testing)
- Plolia injections (PT provides medication)
- B-12 injections
