- Submission of completed registration application required from parent(s) and from health professional
Eligibility / Target Population
- Children and youth 18 years of age and younger with permanent physical disabilities
- Incontinence Supplies Grant Program available for families with children living with a chronic disability resulting in irreversible incontinence or retention problems lasting longer than six months

Description of Services
Provides programs and services to children, youth and young adults with physical disabilities, including:
- Funding for essential mobility and accessibility equipment including but not limited to wheelchairs, walkers, porch ramps/lifts, bath and toileting aids, and communication devices.
- Accessible summer camp programming for children, youth and young adults with physical disabilities. Camp properties: Camp Merrywood (Perth, ON) and Camp Woodeden (London, ON)
- Special education resources for parents.
- Post-secondary scholarships for students with physical disabilities.
- Administers the Incontinence Supplies Grant Program on behalf of Ontario Health
