Description of Services
PRC's (Psychogeriatric Resource Consultant Program) provide education and consultative support to team members at Long-Term Care Homes, Hospitals and Ministry of Health funded community support agencies that provide care for older adults with neurocognitive disorders (e.g. dementia, delirium), other complex physical and mental health needs and associated responsive behaviors.
PRCs support training on the completion of screening tools, and provide guidance on which screening tools and interventions strategies to use based on the needs of the individual. PRCs work as advisors in partnership with existing formal supports to guide and empower teams in problem-solving and solution-finding through knowledge transfer and indirect consultation.
