Mental health service that is delivered by a multidisciplinary health team to initiate treatment, rehabilitation, and support services to accommodate a client's objectives * service is for those who have serious and persistent mental health illnesses

- individualized recovery focused treatment/counselling
- clinical psychiatric treatment
- crisis prevention, intervention and management

- health promotion and teaching
- referrals to other community resources
- coordination of support
- service delivery in community locations
