Description of Services
This program provides behavioural healthcare services for older adults in Ontario with, or at risk of, responsive behaviours/personal expressions associated with dementia, complex mental health, substance use and/or other neurological conditions. This includes adults with age related neurocognitive conditions (such as early onset dementia) and support for family and professional care partners.
Clinical Behaviour Response Specialists: Provide direct clinical assessment, treatment, transitional support and behavioural health care planning to explore potential causes of responsive behaviours/personal expressions to develop formalized care strategies with care partners.
Behavioural Support Facilitators: Support the family care partner(s) of individuals being supported through BSO by providing one-on-one coaching, support, reflective goal-setting and assistance with system navigation.
Psychogeriatric Resource Consultants: Support healthcare providers in long-term care homes (LTCHs), Home and Community care, hospitals, and MOH funded community support organizations that provide care for older adults with or at risk of, responsive behaviours/personal expressions associated with dementia, complex mental health, substance use and/or other neurological conditions.
Dedicated BSO Staff in Long Term Care: Provide coaching/mentoring, behavioural assessment and individualized non-pharmacological care strategies and are based in various long-term care homes throughout the North East.
