A group of family physicians at 3 sites who provide primary care services * medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family physicians, nurse practitioners, nurses, pharmacists, dietitians, social workers, occupational therapist, physiotherapist, psychologist and psychiatrist

Several chronic disease management programs including, but not limited to, Diabetes Education, Newborn Support and Nutrition Education

In addition to program-based care, patients receive clinic-based primary care from HealthLink Family Practice and the Hespeler Medical Clinic
