Patients at all South Bruce Grey Health Centre hospitals are screened for program eligibility once they have completed the acute portion of their stay and are awaiting discharge to an alternate destination
Eligibility / Target Population
Geriatric patients who are no longer being treated for an acute illness and are awaiting discharge to an alternate destination, such as long-term care, or home with community supports
