Referral by local Physician and a Registered Dietitian
Eligibility / Target Population
Patients 18 years of age and older of the Family Health Team who meet three or more of the criteria for Metabolic Syndrome including the following:
- High blood sugars
- Abnormal cholesterol
- High blood pressure
- High triglycerides
- High abdominal body fat

Lifestyle intervention program
- Offers a six-month program offered by a team of healthcare professionals including a Dietitian and Physiotherapist
- Offered in a group setting of 15 participants
