Description of Services
A specialized outpatient program to support individuals with wounds that are slow to heal
- Supports the physical, mental, emotional, environmental, nutritional and financial needs of the individual
- Provides consultation with other healthcare providers as needed
- Provides Telemedicine appointments as appropriate
- Offers a multidisciplinary team consisting of Nurse specialized in wound, ostomy and continence, Physiotherapist, Occupational Therapist, Dietitian, Social Worker, Chiropodist, and Physician
