Referrals from a physician or nurse practitioner for this service should be initiated through our Central Navigation.
Eligibility / Target Population
Adults over the age of 65 with a diagnosis or suspected diagnosis of depression, bipolar mood disorder, mood disorders associated with neurological conditions, mood disorders associated with bereavement, or anxiety. Patients are unable to attend onsite appointments at Baycrest or require regular case management

Treatment programs for people living with late life mental health challengesOur interprofessional services include:
- Assessment and diagnosis
- Treatment including medication management and non-pharmacological approaches
- Case management including: advocating for clients and families
- Monitoring and ongoing support of clients
- Referrals and coordinating access to other Baycrest services and/or community programs

Appointments to be held at the patients’ home.
