People whose primary concern is problematic substance use * must have a history of repeated entries into treatment, have a clear commitment toward abstinence, have been unable to attain or maintain housing without support, be capable of living independently with support, and agree to complete a comprehensive assessment process

Case managers help clients with problematic substance use find independent housing with peer support * offers transitional housing in a home environment for up to one year * case managers help clients work on skills in daily living, coping strategies, and new, healthier behaviours
